GDPR

  1. We kindly inform you that sole proprietorships operating under the SysMedic.pl brand: Bok 11 Solutions Wojciech Kaczmarek, with its registered office at ul. Sisterzana 7/5, 53-029 Wrocław (NIP: 883-112-81-84) and IT-Doc Maciej Urbaniak, ul. Jerzego Bajana 69/15, 54-129 Wrocław (NIP: 692-224-86-51) process your personal data within the meaning of the Regulation of the European Parliament and of the Council of the European Union 2016/679 of 27 April 2016 on the protection of natural persons in in connection with the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter: "GDPR").

  2. In order to fulfill the information obligation specified in art. 13 GDPR, we inform you that:

  5. The administrator has not appointed a Data Protection Officer,

  6. Your personal data is processed to the extent necessary to provide IT care services, ad hoc IT support as well as IT consulting and other related / related services.

  7. The processing of the Customer's personal data may concern his e-mail address, name and surname, telephone number, address of residence or business address and computer IP address.

  8. The data administrator uses technical and organizational measures to ensure the protection of personal data being processed, as specified in art. 25, 30, 32-34, 35-39 of the GDPR ensuring increased protection and security of the processing of Customers' personal data, appropriate to the threats and categories of data protected, in particular technically and organizationally securing data against disclosure to unauthorized persons, removal by an unauthorized person, processing in violation of the Act and change, loss, damage or destruction, including SSL (Secure Socket Layer) certificates and disk and other media encryption are used. The set of collected Customers' personal data is stored in a secured and encrypted IT system.

  9. The legal basis for the processing of your personal data may be your consent (Article 6(1)(a) of the GDPR), the need to take action at your request (Article 6(1)(b) of the GDPR) or purposes related to legally legitimate interest pursued by the administrator or by a third party (Article 6(1)(f) of the GDPR). The Administrator's legitimate interest is the need to maintain effective business contacts with customers in order to provide effective IT care to the Customer.

  10. Your personal data will be processed by the Administrator for the period necessary to provide services or for other purposes indicated in point 4 above, then they will be deleted, unless further processing results from another legal basis.

  11. You have the right to:

  • request from the Administrator access to your personal data,

  • request the Administrator to correct their personal data,

  • request the Administrator to delete or limit the processing of your personal data,

  • raise an objection to the Administrator against the processing of personal data for direct marketing purposes,

  • raise an objection to the Administrator against profiling to the extent that the processing is related to direct marketing,

  • withdrawal of consent to the processing of your personal data,

  • lodge a complaint with the supervisory authority,

  1. Please be advised that you have no statutory obligation to provide personal data, however, this obligation may result from specific provisions. The consequence of not providing personal data may be, in particular, the inability to provide services to you.

Please be advised that your personal data will not be made available to other third parties, excluding sole proprietorships operating under the SysMedic.pl brand, hosting companies and telecommunications operators whose services are used by SysMedic.pl, entities providing accounting services for companies operating under the SysMedic.pl brand .

  3. We reserve the right to disclose, in justified cases, selected information regarding your personal data to the competent authorities or third parties who submit a request for such information, based on an appropriate legal basis and in accordance with the provisions of applicable law.
